Things to expect during your visit: review of your health history, medication review, vital signs, physical exam, recommendations for prevent measures, and bloodwork.

Sick Visits
Generally focus on addressing a specific illness or symptoms you’re experiencing. During this visit, illness or symptom history is taken, vital signs, physical exam, confirmatory testing and possibly bloodwork. Diagnosis and treatment plan are the goals during this visit.
Chronic Care Management
Chronic conditions are health conditions that last extended periods of time and require ongoing medical care. Some of these conditions include hypertension, diabetes, high cholesterol, heart disease, COPD, etc. Chronic condition appointments typically include blood work for monitoring, medication review for compliance, physical exam, vital signs, and discuss lifestyle modifications. Setting health goals and establishing a follow up plan is the best way to keep your chronic conditions controlled.
